After-School Event Permission Form and Guidelines

5th-8th Grade
Date: Friday, May 1, 2026
Time: 5:30 - 7:30 p.m. (ASB Only: Dance is from 5:00 to 8:00. Time includes set-up and clean-up.)
Place: Quantum Academy Great Hall
Price: Free to fifth, sixth, seventh, and eighth-grade students who demonstrate safe, responsible, and respectful behaviors in accordance with our school’s guidelines. Snacks and drinks will be sold at the event for $1.Thank you, PTA, for sponsoring this event!
After-School Event Guidelines:
Quantum After School Events are for Quantum Academy students only.
Students are to remain inside the school during the event until the event ends or a parent or guardian picks them up.
Parents or guardians must pick up their students within 15 minutes of the end of the event. A parent or guardian with transportation must be available by telephone for students attending the event.
Students not attending school events may not be on the school grounds during these events.
All school rules apply before, during, and after any school events.
Appropriate school attire must be worn. If you have any questions, please call the school at (760) 432-2220 and/or review your student’s planner.
Students must abide by the District’s Board Policy on the use of electronics at school events, including cell phones and personal cameras. Phones will be collected upon entering the dance and kept secure. Phones will be returned after the dance. If you have an emergency and need to reach the school, please call: (760) 432-2220
Students must be safe and responsible during the event. Parents or guardians may be called to pick up students who are not being safe and responsible.
Students may NOT bring outside foods or drinks.
Students should not have any ODR’s during the week of the movie in order to attend.
Acknowledgment of this document means you have read the after-school event guidelines, and you agree to support these rules. You are also acknowledging that you agree to support the school by holding your student to complying with these guidelines and giving permission for him/her to attend this event at Quantum Academy.
